Neal Adams Portfolio
According to ComicArtFans.com, this portfolio was never produced, and this plate is the only one that ever existed.
Neal Adams was also to do an X-MEN Marvel Graphic Novel, but he withdrew from the project when he said that he did not receive the contract he was initially promised by Jim Shooter, sometime in early 1983. He probably declined to do the X-MEN portfolio as a result. The X-MEN graphic novel became the X-MEN: GOD LOVES, MAN KILLS graphic novel by Chris Claremont and Brent Anderson, published in April 1983.
I was wracking my brain trying to recall where the above ad was published. I looked through my 1981-1982 COMICS JOURNAL issues, and didn't see it there. I'd guess this above ad appeared in a 1982 issue of AMAZING HEROES.
